This is the date that the actual incident occurred. When recording the date, please ignore earlier events that may have contributed to the incident.

For example, a patient is prescribed anti-hypertensive medication at an excessive dose. As a result she becomes dizzy and falls.

For the purpose of this question, you would ignore the date on which the treatment was prescribed and document the date of the fall.
