Please use only one of the following options to add the age of the patient to the incident

The patient's age can be inserted using either the date of birth or the age (years, month and /or weeks). If you do not know the age of the patient select "unknown". If entering the actual age of the patient this should reflect the age at the date the incident occurred.

If reporting patient age at the time of incident in weeks, please use the following for a guide:

0 weeks = 0 days to 6 days
1 week = 7 days to 13 days
2 weeks = 14 days to 20 days
3 weeks = 21 days to 27 days

Please remember that day 0 is the date of birth
